CommuniCare Health Centers is a private, non-profit health care organization offering comprehensive medical and dental services, vision care, substance abuse treatment, health education and outreach services. CommuniCare provides services at five community clinic sites located in Davis, Woodland, Esparto and West Sacramento, and outreach services throughout Yolo County.
Since 1972, CommuniCare has been promoting healthy communities by providing affordable and culturally competent health care for those in need. CommuniCare is a Federally Qualified Health Center (FQHC).
