Clinic Locations CommuniCare Health Centers operates six clinics at sites located in four towns and cities throughout Yolo County. Click on the links below for information about each clinic site, including a location map, phone numbers, address, clinic hours of operation and services available at the clinic. Primary medical care, health education and outreach services are provided weekdays at four clinic sites, located in Davis, Woodland and West Sacramento. Dental services are offered on a schedule that rotates services between clinics in Davis, Esparto, Woodland and West Sacramento. Substance Abuse Treatment services, as well as health education and outreach services are provided at both John. H. Jones Community Clinics.
