CommuniCare Health Centers offers benefits to eligible employees
working 20 or more hours per week. For more information, contact the

Optional Benefits:
Vacation - CommuniCare
Health Centers offers vacation benefits to employees working 20 or more hours per week. A full time regular
employee receives 3.08 hours per bi-weekly payroll (two weeks) for the
first two years, 4.62 hours per bi-weekly payroll (three weeks) for the
next two years, and 6.15 hours per bi-weekly payroll (four weeks) for 5
or more years of employment. Vacation begins accruing with the first
day of employment, however, cannot be used until completing the six
month introductory period.

Sick Leave -
CommuniCare Health Centers offers sick leave benefits to employees working 20 or more hours per week. A full time
regular employee receives 3.69 hours per bi-weekly payroll or one day per month. Sick leave
may be used during the six month introductory period. Employees will
not be compensated monetarily for accumulated sick leave upon
termination of employment.

Holidays -
CommuniCare Health Centers offers 11-1/2 paid annual holidays to
eligible employees. To qualify for a paid holiday, the employee must be
benefit eligible and must be normally scheduled to work on a day that
falls on a holiday.

Floating Holiday -
CommuniCare Health Centers offers a floating holiday to employees working 20 or more hours per week. A full time
regular employee receives 8.00 hours per year. Part time employees
working between 20-39 hours per week will be prorated. This is a “use
it or lose it” benefit. The float hours are provided every July 1 and
must be used by June 30 of the following year. This benefit is not
provided to midwives (see PP&PM).

Health Insurance
- CommuniCare Health Centers offers employees a choice between two
Health Maintenance Organizations (HMO): Kaiser and PacifiCare. There is
no cost to the employee for health insurance. Employee’s can enroll
family members through payroll deductions on a pre-tax or after-tax
basis. Contact the Human Resources Department for the monthly benefit
rates for dependents (spouse/children).

Dental and Vision Insurance
- CommuniCare Health Centers offers employees dental & vision
insurance through PacifiCare. There is no cost to the employee for
dental & vision insurance. Employee’s can enroll family members
through payroll deductions on a pre-tax or after-tax basis. Covered
dental benefits will be paid up to $1,500 per calendar year. Covered
orthodontic benefits will be paid up to $1,500 per lifetime. Contact
the Human Resources Department for the monthly benefit rates for
dependents (spouse/children).

Pension Plan - After
completion of one year of employment and 1,000 work hours, CommuniCare
Health Centers will contribute an amount equal to 5% of the employee’s
total gross wages into the Pension Plan. The contribution is not vested
until completing 5 years of employment. Please note, the 403(b) and the
Pension Plan are two separate plans. This is not a match.

Life Insurance -
CommuniCare Health Centers offers benefit-eligible employees $10,000
basic life insurance, $10,000 accidental death & dismemberment, and
$1,000 basic life for dependents through PacifiCare.

Paid Time Off (physicians only) -
CommuniCare Health Centers offers paid time off benefits to physicians
working 20 or more hours per week. Paid time off includes vacation,
sick leave, and continuing education. A full time regular physician
accrues PTO as follows:
0-24 months: 5.19 weeks/year = 27 days (216 hrs) or 8.31 hr bi-weekly
25-48 months: 6.15 weeks/year = 32 days (256 hrs) or 9.85 hr bi-weekly
49+ months: 7.12 weeks/year = 37 days (296 hrs) or 11.38 hr bi-weekly
PTO begins accruing with the first day of employment;
however, PTO cannot be used during the first ninety (90) days of
employment. For more information, contact the Director of Human

Optional Benefits:
Premium Only Plan/Dependent Care
- CommuniCare Health Centers offers employees the option of paying
monthly health and dental premiums as a pre-tax or after-tax benefit
utilizing a third party administrator (TPA). This provides the employee
with a means to have less “taxable” wages while not reducing “gross”
wages. The TPA also provides a Flexible Spending Account (FSA) which
allows the employee to have dependent care expenses deducted on a
pre-tax basis. This benefit is available if the employee is paying for
dependent care for children under the age of 13.

403(b) - After
completion of the six month introductory period, CommuniCare Health
Centers allows employees to participate in 403(b) salary deferrals.
Employees can defer as little as $25 per month and as much as $13,000
annually (indexed up each year to the Internal Revenue Service’s
maximum allowed amount). This is a great way to save for retirement and
is always 100% vested. This benefit is available to all employees of
CommuniCare Health Centers.

Davis Athletic Club -
CommuniCare Health Centers has joined the Davis Athletic Club. This
means the one time initiation fee for membership has been reduced to
$30. Employees can join the Davis Athletic Club through payroll
deductions of $47 per month for one, $96 per month for two, $106 per
month for a family of four (2 adults, 2 children--ages 13 to 18).
Family members must be 13 or older to participate.

Yolo Federal
Credit Union - CommuniCare Health Centers is a member of the Yolo
Federal Credit Union. Therefore, employees are able to join the credit
union utilizing its services (i.e., checking, savings, loans, etc.)

Pre-Paid Legal Services
- Employees have the option of purchasing prepaid legal services for
$15.95 per month. This is an optional benefit that can start and stop
at your discretion.
