Community Health Education Program (CHEOP)
CommuniCare Health Centers’ program for chronic disease management and obesity includes in-clinic medical services for patients living with diabetes, hypertension, asthma, and high cholesterol, as well as an education-based program to support improved self-managed care.
Our Community Health Education Outreach Program (CHEOP) supports the special needs of Yolo County’s culturally diverse populations who are living with, or at risk of developing, chronic diseases. The CHEOP program promotes better health for clients by reducing cultural barriers to care, coordinating patient care through CommuniCare medical services, providing group and individual education, and encouraging clients to improve their self-care for better long-term health outcomes.
