One of the best and most cost-effective ways to prevent disease and promote healthy communities is to teach people how to take good care of themselves and their families. CommuniCare Health Centers provides comprehensive health education and outreach services throughout the county.
Health education, nutrition and coordination of medical care are provided to clients with chronic diseases of asthma, hypertension and diabetes. Multilingual staff works in homes and community settings to provide comprehensive case management.
HIV/AIDS Program
Prevention education and HIV testing are provided at detention centers, alcohol and drug treatment centers, Juvenile Hall, recovery homes and on the streets. Support services and coordination of care are provided for HIV positive and AIDS patients.
Teen Clinic
Medical services specifically targeted for teens are delivered during special in-clinic hours.
Teen Program
Presentations and Peer Counseling that focuses on reproductive health, health education and positive life skills are specifically geared for teens and offered at school and community sites.
